The eCHECKUP TO GO is designed to provide you with personalized information and feedback regarding your alcohol use, and how it might affect your health, your relationships, and your career and life goals.

Although you will be asked a number of questions, the program is not intended to be a survey or questionnaire to get information from you.

Instead will be given an opportunity to reflect on your alcohol use and your responses will help generate your personalized feedback report.

All the information you provide is confidential, and unless you choose to share your personal information at the end of the program, it is also anonymous.

Should you want to notify someone that you have completed the program, you will be able to do so at the end.
